ON LINE ASSESSMENT FORMS:
Responding to an "on-line" assessment is somewhat different from filling out
a paper form, but the goal is the same; to accumulate accurate information.
IMPORTANT: When responding to the assessment program use only a mouse or similar pointing device. Do not press the "enter" key on your keyboard as it will terminate the program.As you navigate through the assessment process you will encounter three different types of entry requirements. Here are some examples:(Go ahead, try them out)
1. A small "radio" button. Click on the one with the correct answer.
Example:

2. A text box. This is generally a blank rectangle that requires you to enter text. Click on the box and begin to enter your response.
Example:
What is your name?
3. A rectangular "selection" bar with a down arrow inside. Click on the arrow and select one answer from several choices.

IMPORTANT NOTE:As you take the assessments, each time you progress from one page to another you will be asked to enter your name and the name of the company that requested you to participate in this assessment program. We need this information to make certain each page is properlty labeled. Kindly fill out each form carefully and completely. Thank You
